Need a Doctor’s Note for Work? What to Do Today

If you feel too sick to go in, stay home. Call 911 for chest pain, trouble breathing, a sudden stroke sign, heavy bleeding, or if you cannot stay awake. You almost never need an emergency room just so your employer can have a note.

Call 911 first if you have any of these

Call 911 or go straight to an emergency room if you have:

  • Chest pain or pressure
  • Trouble breathing
  • Face droop, arm weakness, or slurred speech
  • A seizure, fainting, or you cannot stay awake
  • Heavy bleeding you cannot stop
  • A severe allergic reaction

Most sick days are not that. A fever, a cough, a sore throat, or a stomach bug is not a 911 call.

If your city offers TAP, you can text a clinician instead of waiting in urgent care for something a phone visit can handle. Start at taptelehealth.com/text-a-doctor. TAP currently serves residents of Algonquin, Illinois; Chesterfield, Cheraw, Pageland, McBee, Jefferson, Mount Croghan, Patrick, and Ruby, South Carolina; and Crandall, Henderson, Ferris, Mabank, Paradise, and Troup, Texas, plus Texas Health Benefits Pool members. Check the list on that page for your city.

Do you actually need a note?

Your employer writes the rule. Some want a note after one missed day. Some wait until day three. Some never ask. Read the handbook or ask your manager before you book a visit. A note is paperwork, not emergency care, and it does not require an ER trip.

How to get a note without an ER trip

Once you know a note is required, use the simplest option that still gets you seen:

  1. Your own primary care office, if they have a same-day slot
  2. Urgent care, if the office is closed and you need an in-person look
  3. A telehealth visit, if a clinician can handle it by phone or text

Urgent care is for problems that cannot wait and are not 911. Telehealth is for questions a doctor can answer without an exam in the room, like a straightforward cold, a UTI, or a medication refill.

If your city offers TAP, that is the use case. Text a doctor at taptelehealth.com/text-a-doctor, describe your symptoms, and ask for a work note if the clinician agrees you need one. Live cities are listed on that page, including Algonquin, Illinois; several South Carolina towns around Chesterfield County; and several Texas cities plus Texas Health Benefits Pool members.

What a work note usually says

Most employers want three things on the note:

  • That you were seen, or advised to stay home
  • The date you should stay out
  • The date you can return

They rarely need a diagnosis. Do not dump the details in Slack or email. Give HR the note and keep the rest private.

If you cannot get in today

Tell your employer you are out sick. Ask in writing whether this absence needs a note. If they say no, rest. If they say yes, take the earliest slot you can find and send HR the appointment time so the delay is on the record.

If your city has TAP, texting a clinician is usually the fastest way to get both an answer and a note. Start at taptelehealth.com/text-a-doctor and check whether your city is on the live list.
